How to prepare for your first appointment:

1.) Paperwork:

‍ ‍a.) You will receive an email with a link to your client portal. This is where you can complete any necessary paperwork.

b) Please complete paperwork, upload your insurance card and payment information.

c) If I accept your insurance, I will submit these claims. If I am an out-of-network provider for your insurance company, I will provide you with all necessary information for you to submit to your insurance company. If you do not have insurance and are self-pay, payment is required at the time of service. Thank you.

d) The day of your first appointment, you will receive an email and text link to your virtual appointment. Please access this link no more than 10 minutes before your scheduled appointment.
